About The Position

The Ohio Department of Behavioral Health (DBH) is seeking a Licensed Practical Nurse (LPN) to join their team at the Northwest Behavioral Healthcare Campus in Toledo, Ohio. This role involves providing medication, education, and empowerment to individuals, observing and recording patient behavior and response to medication, monitoring patient health including vital signs and treatment plan progress, and serving as a member of a multi-disciplinary team. This is an hourly position covered by the OCSEA/AFSCME bargaining unit. The State of Ohio is committed to disability inclusion and is a drug-free workplace. Applicants must possess a valid LPN license in Ohio and proof of completion of an approved course in medication administration. Official transcripts for post-secondary education are required.

Requirements

  • License to practice nursing as a licensed practical nurse issued by Ohio Board of Nursing pursuant to Section 4723 Ohio Revised Code
  • Proof of completion of an approved course in medication administration.
  • Licensed Practical Nurses who administer dialysis treatment must possess Certification in IV Therapy pursuant to Section 4723.17 Ohio Revised Code.
  • Official transcripts are required for all post-secondary education, coursework, or degrees listed on the application.
  • Applicants must submit an official transcript before receiving a formal employment offer.
  • Failure to provide transcripts within five (5) business days of the request will result in disqualification from further consideration.

Responsibilities

  • Provide medication, education, and empowerment to individuals
  • Observe, evaluate, and record patient behavior, condition, and response to medication.

  • Serve in a fundamental role as a multi-disciplinary team member that creates a safe and positive environment through safety and observation rounds and group facilitation.
