About The Position

Providence Health Care is accepting applications for Licensed Practical Nurses (LPNs) to join their harm reduction-driven team at Crosstown Clinic. This clinic is the first in North America to offer medical-grade heroin (diacetylmorphine) and the legal analgesic hydromorphone within a supervised clinical setting to chronic substance use patients. The clinic is supported by an on-site pharmacy and an interdisciplinary care team composed of doctors, nurses, social workers, and substance-use disorder counsellors. Services also include life-skills counselling, housing referrals, and direction to legal assistance. The LPN reports to the Patient Care Manager or Resident Care Manager and works collaboratively with the interdisciplinary team. The role involves providing nursing care to an assigned group of patients/residents according to established policies and procedures and the Standards of Practice for the Licensed Practical Nurses of British Columbia. The LPN functions as an independent practitioner, recognizing when to access the RN/RPN for collaborative support or clinical intervention based on patient acuity or scope of practice.

Requirements

  • Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
  • Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
  • Current basic cardiac life support certification.
  • Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
  • Physical ability to carry out the duties of the position

Responsibilities

  • Assists patients/residents in meeting physical, social, spiritual, and psychological needs

  • Participates in care planning, assessments, interventions, and evaluations
  • Documents information and advocates for patients/residents
  • Attends various meetings including care conferences
  • Assists with orientation of new staff and students
  • Responds to and reports incidents following established procedures
  • Performs clerical duties as required
  • Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities.
  • Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed.
  • Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team.
  • Administer medications per protocol, monitor effects, and maintain communication with the team.
  • Utilize evidence-based resources to stay updated on nursing practices.
  • Respond to incidents such as codes, injuries, spills, and alarms, following procedures.
  • Assist with staff orientation, demonstrating procedures and providing support.
  • Address patient/resident and family learning needs, provide educational materials, and make referrals.
  • Attend care planning meetings, provide input, and advocate for patients/residents.
  • Collaborate with the team to develop and evaluate individualized care plans.
  • Assist with discharge planning based on patient status and external supports.
