About The Position

Active Day is seeking a Licensed Practical Nurse (LPN) to provide nursing care to members, making a difference in their lives. Unlike traditional hospital or residential care settings, this role offers a more manageable workload with opportunities to build relationships with members and provide person-centered care. You will rarely handle acute care conditions and will have support staff, including aides, to assist you. This position offers full-time, part-time, and per diem options, with the benefit of not working 12-hour shifts, being on call, or facing unpredictable patient ratios.

Requirements

  • LPN License
  • CPR certification
  • Valid malpractice insurance policy if applicable
  • At least 21 years of age
  • At least one-year full-time equivalent experience in a health care setting preferred
  • Prior experience working seniors and/or adults with special needs preferred
  • Strong communication and engaging interpersonal skills
  • Able to multi-task and manage time effectively
  • Able to work a flexible schedule
  • Able to bend, pull, push, squat, turn, transfer, and lift 50 pounds; able to stand for an extended period while providing physical assistance to members
  • Successful results of background check, and where applicable, additional state-specific regulatory requirements

Responsibilities

  • Provides nursing care to members according to individual plan of care, physician's orders and emergency needs.
  • Observes, monitors and documents members' vital signs and responses to treatments and medications, reporting abnormalities to Director of Nursing.
  • Assists members as needed with daily functional activities, including but not limited to, hygiene, meals, ambulation, arrivals and departures, and social activities.
  • Provides health education and instruction to staff, members and family members.
  • Provides emergency care as required.
  • May be required to administer medications with appropriate certification and supervision by the Registered Nurse.
  • Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large.
  • Participate in staff meetings and in-service educational programs as requested; actively collaborate with cross-functional team, providing feedback and recommendations for enhancing the member experience and achieving the center’s overall goals and objectives.
  • Comply with company policies and procedures, safety and regulatory laws and standards.
  • Other duties and responsibilities as changed or assigned at any time.
