About The Position

The Licensed Practical Nurse supports safe, consistent, resident-centered care in a post-acute care setting. Working the 7 a.m.–7 p.m. day shift in Meridian, Mississippi, the LPN delivers hands-on nursing services, maintains clear clinical documentation, and helps coordinate care with CNAs, therapy staff, clinical leaders, providers, residents, and families. The role includes administering ordered treatments and medications, completing and updating admission and ongoing assessments, contributing to individualized care plans, and supervising and supporting CNAs, including providing input to performance evaluations. The LPN also monitors and documents fall-prevention and skin-integrity/pressure-injury initiatives, promptly escalates changes in resident condition, responds to call lights and new symptoms, and provides an accurate handoff to the next team. The employer provides senior healthcare services, including short-term rehabilitation, transitional care, and Alzheimer’s and dementia care.

Requirements

Active Mississippi LPN license in good standing.
Strong attention to detail, sound judgment, and compassionate communication.

Skilled nursing or rehabilitation experience preferred.

Responsibilities

Deliver hands-on nursing services and maintain clear clinical documentation.
Administer treatments and medications as ordered.
Complete and update admission and ongoing assessments.
Contribute to individualized care plans.
Supervise and support CNAs, including providing input to performance evaluations.
Monitor and document fall-prevention and skin-integrity/pressure-injury initiatives.
Promptly escalate changes in residents’ conditions to appropriate clinical leaders or providers.
Coordinate care with CNAs and therapy staff and respond to resident and family questions professionally.
Reassess residents, respond to call lights and new symptoms, and keep documentation current.
Confirm orders are addressed, notes are complete, and provide a clear, accurate shift handoff.
