About The Position

Catholic Health Home Care is seeking dedicated Licensed Practical Nurses to provide comprehensive skilled nursing care to patients in their homes. The role serves an assigned caseload across the listed Team 1 communities and involves working with patients in varied and potentially complex home situations. LPNs collaborate with an interdisciplinary patient care team and provide patient and family education in accordance with the physician’s plan of treatment. The schedule is five 8-hour shifts, 8:30 a.m. to 4:30 p.m., with rotating weekends. The listed salary range is $33.00–$40.00 per hour. Additional compensation is available for visits over the caseload. Catholic Health describes its pay range as a good-faith estimate; actual pay may depend on qualifications, skills, competencies, and experience, and the stated range excludes applicable bonuses, incentive or differential pay, other compensation, and benefits.

Requirements

Current New York State Licensed Practical Nurse (LPN) license.
At least one year of recent hands-on experience.
Basic Life Support (BLS) certification.

Responsibilities

Case manage, assess, plan, develop, and implement comprehensive home health care plans for patients.
Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments to varied patient populations in potentially complex home situations.
Work with physicians, therapy and social services, pastoral care, home health aides, and other members of the patient care team for the assigned caseload.
Provide patient and family education on disease management and health promotion as directed by the physician’s plan of treatment.
