About The Position
Traditional Home Health Care Pediatric Division is seeking a compassionate Licensed Practical Nurse (LPN) to join their team. They provide exceptional skilled nursing and aide services to pediatric and adult patients in their homes, aiming to restore health, build confidence, and promote independence. This role offers meaningful one-on-one patient care, flexible scheduling, and the opportunity to make a significant difference in patients' lives.
Requirements
- Licensed Practical Nurse (LPN)
- Compassionate
- Ability to provide one-on-one patient care in a home/school environment
- Ability to implement and document the plan of care
- Ability to administer prescribed medication, treatments, and therapies
- Ability to perform patient assessments and coordinate care
- Ability to report changes in client condition to Clinical Manager and physician
- Ability to provide health promotion, teaching, and training of family members
- Must be comfortable with large dogs in a home environment (for Cresco, PA position)
Nice To Haves
- Experience with Tracheostomy (TRACH)
- Experience with Ventilator (VENT)
- Experience with Gastrostomy Tube (G/J Tube)
- Experience with G-tube care
- Experience with Trach/Vent care
Responsibilities
- One to One patient care in home/school environment
- Implement and document the plan of care to ensure quality and continuity of care for patient
- Administration of prescribed medication, treatments and therapies
- Patient assessments; coordination of care
- Reports any changes in the client condition to Clinical Manager and physician
- Health promotion, teaching, and training of family members
