About The Position

The Licensed Practical Nurse (LPN) will be responsible for medication administration for residents in compliance with Federal & State regulations. This role may involve contributing to the admission of new residents, including assessments and documentation. The LPN may also contribute to the implementation of care plans and the collection of data. Additionally, the LPN might be responsible for clinical system tracking, such as wounds or infection control, under the direction of the director. The position involves providing general consultation on health-related concerns within the community, notifying physicians and family members of changes in resident health, and ensuring proper documentation. The LPN will respond to emergencies, maintain communication with all departments, and provide educational meetings for residents and staff. Understanding the role in the Safety & Disaster Plan, maintaining resident confidentiality, and attending required training are also key aspects of this position. The LPN is expected to strive for a safe working environment and maintain a positive and professional demeanor.

Requirements

Ability to commute/relocate to Clinton, CT 06413 (Required)
1 year of experience
LPN License

Responsibilities

Responsible for medication administration for the residents in compliance with Federal & State regulations
May need to contribute for admissions of new residents to assist all admission assessments and documentation.
May contribute to process of implementing acute and long-term care plans or service plans and/or the collection of data
May be responsible for clinical system tracking (i.e., wounds or infection control) under the direction of the director.
Provide general consultation regarding health-related concerns within the community.
Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
Respond quickly to all emergencies
Maintain close communication with all departments and staff members.
Provide educational meetings for residents and staff.
Understand roll in the Safety & Disaster Plan.
Maintain confidentiality of residents’ and prospective residents’ information.
Attend all required training, in-service, and staff meetings.
Strive to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
Maintain a positive and professional demeanor toward residents, visitors, families, and co-workers.
Perform other duties as assigned
