About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide one-on-one private duty nursing care for a 16-year-old female child in a supportive home setting. This role offers the opportunity to build meaningful connections, deliver personalized care, and witness the direct impact of your work. The position focuses on quality care and becoming a trusted part of the child's life.

Requirements

• Current LPN (or RN) license in the state of Pennsylvania
• Graduate of an accredited nursing program
• At least 6 months of nursing experience (home health preferred, but not required)
• CPR certification
• Reliable transportation and a valid driver's license
• A caring heart, strong independence, and a passion for pediatric care

Responsibilities

• Assist with Activities of Daily Living (ADLs)
• Provide G-tube care and feedings/aspiration precautions
• Seizure monitoring
• Administer medications safely and accurately
• Communicate clearly and compassionately with the care team and family
• Deliver exceptional, family-centered pediatric care
