We are looking for a compassionate Licensed Practical Nurse to join our growing southern Maine team. This full-time position serves patients throughout York, Wells, and nearby communities. Requirements
- Current and unrestricted Licensed Practical Nurse license in Maine
- Graduate of an accredited practical nursing program
- At least one year of experience in an acute care or comparable clinical setting
- Current CPR certification through the American Heart Association or American Red Cross
- Strong communication, organization, and documentation skills
- Compassionate and patient-focused approach to care
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Ability to travel between patient homes throughout Portland and Cumberland County
Responsibilities
- Provide skilled nursing care in patients’ homes under the direction of an RN and physician
- Follow individualized plans of care
- Monitor patients and report changes in condition
- Administer medications and treatments within your scope of practice
- Educate patients and caregivers
- Complete timely and accurate clinical documentation
- Promote patient safety and independence
- Collaborate with nurses, therapists, physicians, and other members of the care team
- Participate in team meetings and the on-call rotation
