About The Position

SERV is seeking a compassionate and caring Licensed Practical Nurse to support consumers in achieving greater independence and life satisfaction. The role delivers health services to clients experiencing mental and physical conditions associated with mental illness. The nurse uses the nursing process, documentation, and communication skills to collaborate with physicians and other team members, and provides nursing concepts and care plans to clients.

Requirements

• Degree from an accredited school of nursing.
• Valid New Jersey Licensed Practical Nurse (LPN) credential.
• Current CPR/FA certificate.
• Must be at least 21 years old.
• Must successfully complete a physical, drug screen, and background check.
• Valid driver's license in state of residence with a good driving record.

Responsibilities

• Deliver health services to clients experiencing mental and physical conditions associated with mental illness.
• Use the nursing process, documentation, and communication skills to collaborate with physicians and other team members.
• Provide nursing concepts and care plans to clients.
• Provide health education training as assigned.
• Administer medication, collect samples for testing, monitor food and liquid intake and output, prepare injections and enemas, provide patient hygiene, and take vital signs.
• Cooperate with the licensee, Department of Human Services, and staff during inspections, inquiries, or investigations.
• Perform other duties as deemed appropriate or needed by the supervisor.
