About The Position
Grace Community Care & Homes is developing its Home and Community Care department and seeks a Licensed Practical Nurse for private duty nursing. The role contributes to person-centered services, reliable operations, and accurate documentation. The nurse provides assigned care in client homes under required direction and within approved program scope. Services may begin only at approved locations after applicable program approvals, funding, credentialing, staffing, and readiness requirements are met. The position is based in New Jersey, with administrative coordination through Grace and on-site or community assignments as applicable. It is part-time; the assignment and weekly schedule will be discussed during hiring. The start date and duties are contingent on required approvals and staffing readiness.
Requirements
- Active New Jersey LPN license.
- Skills matching the assigned case and approved nursing model.
- Required training and supervision.
- Applicants should provide relevant credentials, experience, availability, and a resume.
- Final regulatory eligibility will be verified before appointment or service delivery.
Responsibilities
- Provide assigned nursing care in client homes under required direction.
- Follow care plans, document services, and escalate clinical changes.
- Collaborate with department leadership, intake, credentialing, billing, and quality teams.
- Maintain confidentiality and respect participant choice.
- Promptly communicate service gaps and follow role-specific policies and professional scope.
- Participate in orientation, competency review, and quality improvement.
