About The Position

Signature HealthCARE of Fentress County is a 140-bed skilled nursing facility providing clinical, rehabilitation, and quality-of-life programs. The Licensed Practical Nurse provides direct resident care, supports residents’ comfort and safety, and coordinates care with nursing assistants, medication aides, physicians, and the interdisciplinary team. The role also includes clinical documentation, care planning, and participation in quality assurance, staff education, and regulatory activities. This is a full-time night-shift role.

Requirements

• Current, active LPN/LVN license in the state of practice.
• Minimum of one year of clinical nursing experience.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of federal and state long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Nice To Haves

• CPR/BLS certification from the American Heart Association or American Red Cross.
• Long-term care experience.
• Experience in a supervisory or charge nurse role.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living and ensure their comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the assigned shift, in accordance with facility policies and regulations as applicable.
• Monitor residents’ physical and emotional status and notify the attending physician and responsible party of changes in condition.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation according to facility policies and procedures.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
