About The Position

The LPN – Advanced Practice is responsible for coordinating care, providing patient education, and managing administrative tasks through remote support for nursing facilities, patients, and caregivers. This role operates under the general supervision of the VP Of Operations, Primary Care, and adheres to organizational policies, federal, state, and accreditation regulations.

Requirements

  • Degree in nursing field as a licensed practical nurse.
  • Minimum of 2 years clinical nursing experience in long-term care, case management, outpatient settings, or similar environment.
  • Must have a current LPN license in good standing in the state(s) in which practicing.
  • Must have excellent communication, comprehension, and interpersonal skills.
  • Must have the ability to speak, read, write and understand English, as well as the ability to make self-understood.
  • Must have strong working computer skills with basic professional computer software (Microsoft Office suite, Windows, etc.) and the capacity for learning new software systems quickly.
  • Must be self-directed with excellent organizational skills and the ability to manage tasks and assignments concurrently.
  • Must have strong motivational skills and a high emotional intelligence quotient (EQ).

Nice To Haves

  • Telehealth or remote nursing experience preferred.

Responsibilities

  • Communicate with nursing facilities to coordinate patient care plans and address clinical inquiries.
  • Provide telephonic support to patients and families, answering questions, educating on care needs, and addressing concerns.
  • Act as a liaison between healthcare providers, nursing facilities, and patients.
  • Review and update patient care plans in collaboration with nursing facilities and providers.
  • Monitor patient progress and follow up to ensure continuity of care.
  • Assist with arranging referrals, home healthcare, or specialized services as needed.
  • Maintain accurate and detailed patient records in the electronic health record (EHR) system.
  • Document all telephonic interactions, care plans, and follow-up actions per organizational policies.
  • Educate patients and caregivers on medication management, treatment plans, and preventative care measures.
  • Provide guidance on chronic condition management and self-care practices.
  • Assist with scheduling appointments, follow-ups, and coordinating transitions of care.
  • Complete pre-authorizations and communicate with insurance providers regarding coverage or care plans.
  • Monitor compliance with regulatory and organizational standards.
  • Identify and address potential barriers to care, escalating issues as necessary.
  • Troubleshoot concerns raised by facilities or patients and provide timely resolutions.
  • Participates in on-call rotation.
