About The Position

This role involves working collaboratively under the direction and supervision of a Registered Nurse, providing comprehensive health care services from patient admission to discharge. The LPN will offer ongoing support and expertise through assessment, planning, implementation, and evaluation of individual patient needs. The primary goals are to enhance patient management and satisfaction, promote continuity of care, and ensure cost-effectiveness. The LPN will also serve as a liaison between patients, family members, and other healthcare personnel to facilitate efficient and appropriate home health care services. Requirements
- Graduate of an accepted Licensed Practical Nursing Program.
- Licensed to practice nursing issued by the state of Virginia.
- Current BLS CPR certification.
- Ability to work independently with minimal supervision.
- Ability to resolve problems and set priorities.
- Excellent written and verbal skills to effectively communicate with confidence, clarity and attention to detail.
- Pleasant telephone demeanor with exceptional customer service skills.
- Basic typing and computer skills.
- Ability to work cohesively with others and multi-task in a busy office setting.
- Current and active driver's license and car insurance.
Nice To Haves
- Minimum of two (2) years experience in home health, acute care or rehabilitation setting.
Responsibilities
- Providing total health care services for patients after admission to discharge.
- Providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs.
- Enhancing the quality of patient management and satisfaction.
- Promoting continuity of care and cost effectiveness.
- Acting as a liaison between patients, family members and other health care personnel to ensure efficient and appropriate home health care services.
- Directing care appropriately to support the specific populations (adult & geriatric) provided in nursing services.
