About The Position

Community Care, Inc. is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide one-on-one private duty nursing care for a 9-year-old male child with trach/vent needs. This full-time role offers a supportive home setting where you can build meaningful connections and deliver personalized care. The position focuses on quality care and becoming a trusted part of the child's life. The schedule involves Monday-Friday day or evening shifts (8:00 AM - 3:00 PM or 3:00 PM - 9:00 PM), with weekend availability.

Requirements

• Current LPN (or RN) license in the state of Pennsylvania
• Graduate of an accredited nursing program
• At least 6 months of nursing experience (home health preferred, but not required)
• CPR certification
• Reliable transportation and a valid driver's license
• A caring heart, strong independence, and a passion for pediatric care

Responsibilities

• Assist with Activities of Daily Living (ADLs)
• Perform g-tube care/feedings
• Perform trach/vent care & suctioning
• Administer Bipap
• Monitor blood sugar
• Monitor vitals
• Administer medications safely and accurately
• Communicate clearly and compassionately with the care team and family
• Deliver exceptional, family-centered pediatric care
