About The Position

The Licensed Practical Nurse (LPN) works with agency clients in the Adult Residential Program, providing medical care and support for self-management. The LPN orients clients to the agency and services, manages their medical care, and facilitates health care services. While residents are responsible for their own health care and medication management, the LPN plays a crucial role in assessing needs, developing treatment plans, educating residents, monitoring medication, and coordinating with external healthcare providers.

Responsibilities

  • Assess potential residents for program appropriateness.
  • Complete updated assessments on each client after admission.
  • Develop a medical treatment/discharge plan based on assessments.
  • Communicate the treatment plan with the treatment team.
  • Provide ongoing health education to residents.
  • Assess acute medical/nursing needs of residents and refer to appropriate treatment centers.
  • Monitor prescribed medication consumption and treatments, assessing resident understanding and accuracy of administration.
  • Coordinate the management of patient medication stock, including pharmacy coordination and refills.
  • Conduct urine drug screens on residents as directed.
  • Conduct weekly psychoeducation classes.
  • Follow treatment care plans set by medical staff.
  • Document all pertinent health-related information in resident charts according to nursing standards.
  • Complete Incident Report forms as applicable.
  • Maintain HIPAA guidelines.
  • Perform other duties as assigned.
  • Review admission packets to determine client eligibility and necessary documentation (PPD, medical records, H&P, Psych evaluation).
  • Determine the need for medical detox based on drug of choice, last use, and consumption level.
  • Meet with new admits to explain MARs, med call schedules, refill processes, and general health conduct.
  • Place and read PPD tests within specified timeframes and refer clients with positive results.
  • Photocopy insurance/Medicaid cards and document emergency contacts.
  • Review charts to determine if patients need to be scheduled for psychiatry.
  • Work with Community Health Center (CHC) staff to schedule psych evaluations.
  • Report appointments and assist in scheduling.
  • Print notes/visits from EHR for client charts.
  • Receive and document verbal orders from providers.
  • Pick up medical slips from the treatment team office twice daily.
  • Take vital signs for patients with medical complaints requiring triage.
  • Coordinate HIV screening with the CHC.
  • Call and coordinate clients to the CHC as needed.
  • Review charts seen by the CHC and follow instructions, including client education on diagnosis and treatment plans.
  • Attend weekly treatment team meetings and present concerns regarding clients.
  • Bring and present the MAR book in weekly treatment team meetings.
  • Ensure clients obtain personal medications upon discharge and sign acknowledgment receipts.
  • Report with night staff for shift change and review night log.
  • Check in with the Front Desk for ER visits after hours and attain paperwork.
  • Ensure medical visits are accompanied by feedback sheets, labs, and notes.
  • Ensure clients report to the nurse's office after medical appointments to report on the visit and submit paperwork.
  • Facilitate medication refills by instructing clients on the process.
  • Call medications into the pharmacy prior to departure.
  • Document pending medications on a pharmacy log.
  • Document arrival of medications into client charts.
  • Insert medication information sheets into patient charts.
  • Send and fill stat prescriptions ASAP.
  • Report clients who do not request medications prior to running out to their Counselor.
  • Ensure all new admits have a medical record in the EHR containing relevant documentation.
  • Administer comfort medications as outlined in policy and instructed by CHC staff or Chief Medical Officer.
  • Document the client's medication schedule and monitor for missed calls.
  • Store medication stock under lock and key.
  • Observe the taking of medication at each medication call.
  • Notify the client's Counselor of missed medication calls after three grace opportunities.
  • Log missed medication calls for treatment team meetings.
  • Instruct clients on medication refill processes and frequency.
  • Submit medication refill requests to Medical Staff.
  • Orient patients to the process of filling out a medical slip when sick.
  • Inform patients that sick visits are triaged and addressed in order of urgency.
  • Consider clients with repeated complaints that impede program participation for discharge.
  • Ensure immediate discharge for all suicidal attempts.
  • Address suicidal ideation directly with OHL Staff and manage client safety.
  • Inform clients of their obligation to stop by the Nursing Office prior to discharge to receive medications.
  • Provide clients with contact information for Behavioral Health and Primary Care Clinics.
  • Review the night log for any ER visits or medical complaints.
  • Review needed actions for the day with the CHC.
  • Review the intake schedule for admissions and prepare appropriately.
  • Review medical slips submitted by clients and triage for urgency.
  • Respond to emails promptly.
  • Coordinate new admits into the CHC for initial H&P and PPD placement.
  • Conduct medication calls.
  • Review Admissions packets and advise on eligibility and documentation for medications.
  • Receive admit charts and review for necessary components.
  • Orient new admits to the medical side of the Residential Program.
  • Coordinate received medications at arrival.
  • Review outside facility medical records for residential clients and determine follow-ups.
  • Email Counselors regarding clients off the floor with new medical orders or pending offsite medical visits.
  • Review Discharge Summary and be aware of clients leaving.
  • Attend Multi-disciplinary Team Meeting and participate in medical components.
  • Coordinate patient referrals with the Psychiatrist and Nutritionist.
  • Follow up on providers’ instructions.
  • Meet weekly with the Community Health Center.
  • Pull discharged clients and note discharge date on a progress note.
  • Manage the MAR daily, weekly, and monthly.
  • Review MAR for patterns of missed medications, inquire with client, and report to Counselor and Behavioral Health.
  • Manage the medications refrigerator and discard old medications.
