About The Position

As a member of the treatment team, the Licensed Practical Nurse will provide direct patient care to assigned patients under the direction and supervision of a Registered Nurse and attending clinician within the Licensed Practical Nurse (LPN) scope of practice. Utilizing the nursing process, the LPN assumes responsibility for contribution of planning, implementing and evaluating nursing care for assigned patients. The LPN plays a key role in patient bedside care: administering medications, wound care, injections, intravenous therapy and other treatment interventions. Provides continuous observation and routine assessment, reporting findings to appropriate members of the patient's treatment team.

Responsibilities

  • Administer oral and intravenous (IV) medications in accordance with the patient's treatment plan and physician orders under the supervision of the Registered Nurse (RN).
  • Assist with IV medication administration and maintenance therapy under the direct supervision of the RN, including: Validating prescribed IV solutions, Assessing and monitoring infusion sites, Administering IV therapy as ordered, Monitoring IV patency and patient comfort, Discontinuing IV therapy at the appropriate time as ordered by the attending clinician.
  • Provide direct nursing care and implement individualized plans of care for assigned patients.
  • Observe and assess patient behavior, symptoms, and responses to treatments and interventions.
  • Monitor and document vital signs, reporting findings and changes in patient condition to the RN.
  • Recognize signs of patient deterioration and collaborate with the RN to increase monitoring, implement appropriate interventions, or facilitate transfer to a higher level of care when necessary.
  • Provide oversight and participate in patient care activities performed by Patient Care Technicians (PCTs), including: Obtaining and documenting vital signs, Assisting with bathing and personal hygiene, Performing linen changes, Monitoring dietary intake, Assisting with ambulation and other basic patient care needs.
  • Collaborate effectively with the interdisciplinary healthcare team to ensure quality patient care.
  • Perform phlebotomy procedures as ordered by the attending clinician, including: Determining the appropriate venipuncture method, Collecting blood specimens according to ordered tests, Verifying patient identification and ensuring proper specimen labeling, Monitoring patients for adverse reactions during and after specimen collection.
  • Conduct bedside glucose monitoring as appropriate.
  • Implement preventive care measures and monitor skin integrity.
  • Perform wound care treatments according to physician orders and individualized treatment plans.
  • Assess wound healing progress and patient response to treatment.
  • Report changes in wound status or patient condition to the RN or supervising clinician.
  • Provide patient and family education based on assessed needs, including: Health conditions and disease management, Medication education, Wellness and preventive health practices, Follow-up care instructions and treatment compliance.
  • Maintain accurate and timely documentation in accordance with organizational policies and regulatory requirements.
  • Perform other duties as assigned by management within the scope of the position.
