The Licensed Practical Nurse (LPN) is responsible for providing nursing care services and case management to clients and families by implementing nursing interventions set forth in the plan of care. This position will start as PRN and will eventually convert to Full-Time. The LPN will provide direct patient care as defined in the State Nurse Practice Act and as ordered by the physician, implement the plan of care initiated by the registered nurse, and provide accurate and timely documentation consistent with the plan of care. The role also involves assessing and providing patient and family/caregiver education, participating in the coordination of home health services, and providing mentoring, coaching, and education to direct care staff under the supervision of the RN. The LPN will use equipment and supplies effectively and efficiently, participate in personal and professional growth and development, and participate in on-call rotation and interdisciplinary team meetings.
