About The Position

Children’s Hospital of Philadelphia (CHOP) is seeking a part-time Licensed Practical Nurse (LPN) for their Kennett Square Primary Care office. This position involves 20 hours per week, with a shift determined by operational needs. The LPN will perform routine nursing care for ill, injured, or infirm patients under the direction of a licensed professional (RN) nurse, nurse practitioner, and/or a physician. The role includes participating in the planning, implementation, and evaluation of nursing care according to the LPN scope of practice and health system standards. Additionally, the LPN will support operations through administrative duties as required, demonstrate competence in caring for patients with moderate to complex health issues, and assist the Site Manager with administrative issues affecting clinical practices within the office. Compliance with administrative policies and clinical protocols is essential. The LPN is administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues and must be willing to rotate to other Care Network sites within the designated cluster if needed.

Requirements

  • Technical Diploma License Practical Nursing Program Required
  • Licensed Practical Nurse (LPN) (Pennsylvania) - Pennsylvania State Licensing Board - upon hire - Required or Licensed Practical Nurse (LPN) (New Jersey) - New Jersey State Licensing Board - upon hire - Required
  • Cardiopulmonary Resuscitation (CPR) - issued through an organization that requires a hands-on instructor lead psychomotor skills verification (ex. American Heart Association or Red Cross). - upon hire - Required
  • Basic use of a computer (mouse, keyboard, printer, USB ports).
  • Basic use of Microsoft Office (Word processing, spreadsheets, presentations and Skype).
  • Web browsing, intranet search, document access.
  • Use of shared network file space.
  • Basic use of patient facing technology. (e.g. IV pumps, point of care devices, CR monitors).
  • Navigation and use of clinical communication systems.
  • Determines the nature and extent of clinical information needed and uses the appropriate technology to access it.
  • Accesses needed information effectively and efficiently.
  • Evaluates information and its sources critically and appropriately incorporates it into clinical practice.
  • Evaluates outcomes of the use of information.
  • Navigation of the electronic health record system.
  • Ability to locate and review specific patient data in various clinical information systems.
  • Effectively uses clinical decision support tools.
  • Understands policies and procedures, confidentiality, security, and privacy regulations regarding the use of clinical information systems.
  • Ability to collect clinical data using quality improvement tools.

Nice To Haves

  • Two years of LPN and pediatric experience is preferred.
  • At least two (2) years experience in the medical field Preferred
  • Pediatric experience Preferred

Responsibilities

  • Provide and document direct nursing care of assigned patients under the supervision of a registered nurse or physician.
  • Nursing care is guided by physician orders and the plan of care. Patient response to care is reported to a registered nurse or physician for evaluation, intervention and modification of the plan of care.
  • Assist other health care personnel in the delivery of patient care.
  • Participate in maintaining the environment of care including equipment and other material resources.
  • Participate in own professional development by maintaining required competencies and attending educational offerings. Supports the development of other staff and formal learners.
  • Participate in the identification of clinical or operational performance improvement opportunities and assists in performance improvement activities.
  • Administer medications that are within their scope of practice and organizational standards only if specific conditions are met (completion of a Board approved educational program, a written order, and knowledge of health system policies and competency training standards).
  • Support clinical activities related to nursing and clinic flow and make recommendations to manager for improvement in quality and effectiveness of nursing care given.
  • Demonstrate commitment to patient safety by speaking up about safety issues and changing nursing practices to enhance safety. Share errors through system reporting mechanisms, and demonstrates shared accountability for safe practices.
  • Perform other related duties incidental to the work described herein.
