About The Position

A Licensed Practical Nurse (LPN) is responsible for LPN nursing care and medical procedures within the scope of the nursing practice. Assist in the integration of physical and behavioral health in program. The LPN reports to the RN and or Nursing Leadership.

Requirements

  • Must hold an active LPN nursing New York State license.
  • Must maintain active CPR certification.
  • Excellent oral, writing, communication and organizational skills.
  • Excellent aural and written communication and collaboration skills.
  • Excellent organizational skills and data management capabilities.
  • The candidate should be able to read documents for analytical purposes such as computer information, software, computer graphic design programs, etc.
  • Limited applications of manual dexterity and hand-eye coordination.

Responsibilities

  • In line with doctor’s orders, administer prescribed and/or routine medical treatments.
  • Ensure that all internal policies regarding such administration and documentation are adhered to.
  • Assist with scheduling appointments and obtaining medical documentation prior to the client’s appointments.
  • Function as the liaison between external medical health care providers, schools, other community organizations and the facility with regards to the client’s medical needs.
  • Provide general nursing care to clients within the scope of LPN practice, which include but are not limited to; Take and record client's vital signs.
  • Administer prescribed/specified mediation and note time and amount on client's chart.
  • Observe clients and report adverse reactions to medication or treatment to supervisor.
  • Phlebotomy and specimen preparation to be sent to the laboratory.
  • Inventory and requisite supplies.
  • Respond to medical emergencies immediately upon notification.
  • Perform whatever triage is necessary to stabilize the client and provide access to all necessary medical treatment within an LPN’s scope of practice.
  • Follow up to ensure complete recovery.
  • Perform all necessary charting and other patient documentation as required by internal practice and external regulatory bodies.
  • Completes all other tasks within the scope of an LPN NYS license requested by the RN and or Nursing leadership.
  • Translate research and other forms of knowledge to improve practice processes and outcomes.
  • Communicate practice knowledge effectively, both orally and in writing.
  • Participate in agency quality improvement projects to ensure best client outcomes.
  • Create a climate of patient-centered care to include confidentiality, privacy, comfort, emotional support, mutual trust, and respect.
