About The Position
Windsor Home Health, a division of PREMIER THERAPY, is dedicated to helping individuals live independent and dignified lives. We are seeking a Full-time Home Health Licensed Practical Nurse (LPN) to join our team and support patients in remaining comfortable in their homes. This role is ideal for an LPN with home health experience who is passionate about making a positive impact on patients' lives through their nursing work.
Requirements
- Proof of graduating from an accredited school of practical nursing.
- Current LPN license from the State of Ohio.
- Current CPR certification.
- A completed and successful TB test, physical examination and a criminal background check.
Nice To Haves
- Previous experience in home health care setting.
- Superior organizational and time management skills.
- Reliable transportation, with a valid driver's license, insurance and registration to travel in assigned territories.
- Demonstrated compassion, empathy and strong customer service skills.
Responsibilities
- Provide nursing care under the direction of the Home Health Registered Nurse and according to the established Plan of Care to achieve positive patient outcomes.
- Communicate with family members and other members of the patient's multi-disciplinary team.
