About The Position

The Licensed Practical Nurse (LPN) provides health services to the students of The Hope Learning Academy Melrose Park. This role involves assisting the RN Case Manager, evaluating health care practices, observing and documenting client health needs, administering medications, performing nursing treatments, and acting as a liaison between clients, staff, and the RN Case Manager.

Requirements

  • Graduate of an accredited school of Practical Nursing.
  • Current licensure in the State of Illinois (or documentation of license pending).
  • Successful background clearance, physical and drug/alcohol screening.
  • A valid Illinois driver’s license and qualify as DCFS licensed driver.

Responsibilities

  • Assists the RN Case Manager in formulating and implementing client health care plans.
  • Evaluates health care practices of the clients and encourages proper care techniques/practices.
  • Observes, identifies, and documents health care needs and problems of clients.
  • Passes medications as required.
  • Correctly prepares field trip medications in a timely manner.
  • Performs nursing treatments, direct care, and other nursing tasks as required.
  • Communicates and documents appropriate information, including new orders, discontinued physician orders, transfers, discharges, admissions, accidents, incidents, injuries, changes in client conditions, new treatments, problems with medications, seizure activity, and other pertinent information.
  • Responsible for primary clients in terms of ongoing monthly physical assessments and weekly documentation of client's general status.
  • Documents observations, treatments, and responses to treatments in a timely manner and in accordance with policy.
  • Acts as liaison between client, staff, and RN Case Manager, notifying the RN Case Manager when appropriate and documenting appropriately.
  • Assists with physical examinations, procedures, or treatments.
  • Carries out physician's orders.
  • Transcribes physician's orders promptly and precisely.
  • Makes rounds on units and/or in classrooms on a regular basis.
  • Attends and contributes to staffings, pre-clinics, and other meetings involving primary clients.
  • Willingly goes to units where needed to pass medications.
  • Demonstrates skills in interpersonal communications.
  • Maintains professional relationships with other members of the Interdisciplinary Team.
  • Actively participates regularly and actively in assigned committee meetings.
  • Maintains familiarity with current nursing policies and procedures.
  • Follows nursing dress code.
  • Identifies potential safety hazards and reports them and willingly keeps the work area clean, orderly, and free of safety hazards.
  • Attends all mandatory in-service meetings.
  • Assumes responsibility as a mandated reporter concerning incidents of purported abuse/neglect of students as outlined in the agency’s policies and procedures.
  • Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
