About The Position
Klarus Home Health is seeking full-time, part-time, or PRN Licensed Practical Nurses (LPN/LVN) to provide community-based care to patients receiving home health services in the Englewood, Aurora, Lakewood, and/or Denver metro area. The LPN provides nursing care to patients of all ages in their residences under the supervision of a registered nurse. The role coordinates care with the interdisciplinary group, patients and families, and referring agencies to support patient outcomes.
Requirements
- Graduate of an accredited School of Vocational/Practical Nursing.
- Current state license as a Licensed Vocational/Practical Nurse, or a qualifying multistate license issued by a state member of the National Nurse Licensure Compact (NLC) when practicing in an NLC member state.
- Current driver's license.
- Two years of experience as a Licensed Vocational/Practical Nurse in a clinical care setting.
- Proficiency in nursing skills as defined by generally accepted standards of practice.
- Good interpersonal skills.
- Reliable transportation and valid, current auto liability insurance.
- Ability to work a flexible schedule and travel locally.
- Ability to lift up to 50 lbs and move patients.

Responsibilities
- Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
- Coordinate care with the interdisciplinary group, patient and family, and referring agency.
- Assist in developing and implementing patient care plans.
- Monitor patient health and report changes to the supervising RN.
- Administer prescribed medications and treatments.
- Educate patients and families about health care needs, conditions, and options.
- Maintain accurate and timely documentation of patient care and progress.
