About The Position

We are seeking a dedicated and compassionate LPN to join the nursing team at Center at Park West in Pueblo, Colorado. The nurse provides high-quality resident care under the direction of attending physicians and the Medical Director. The role includes direct nursing care, monitoring and reporting resident conditions, supporting care planning, maintaining electronic health records, and collaborating with colleagues to provide comprehensive care. The position is in a fast-paced environment with frequent interruptions and may require work beyond normal hours, including weekends and holidays.

Requirements

Current, unencumbered LPN license.
Ability to make independent decisions and exercise sound judgment.
Strong communication and interpersonal skills, including the ability to interact tactfully with residents, families, and staff.
Knowledge of nursing and medical practices, procedures, and long-term care regulations.
Ability to work effectively in a team environment.
Patience, compassion, and a positive attitude.
Ability to lift, push, pull, and transport residents with assistance as needed.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and vital sign monitoring.
Observe and assess residents, and report changes in their condition to physicians and the nursing team.
Assist in developing and modifying resident care plans.
Document resident care accurately and thoroughly in electronic health records.
Collaborate with physicians, nursing staff, and other departments to ensure comprehensive resident care.
Participate in facility surveys and quality improvement initiatives.
Maintain a safe and therapeutic environment for residents.
Supervise other personnel as needed.
