The Ohio State Wexner Medical Center Monday–Friday 8:30 AM -5:00 PM (On call requirements) Looking for More Independence and Meaningful Patient Care? Join our Home Health team and experience the rewarding opportunity to provide one-on-one care while helping patients recover safely and comfortably in their own homes. As a LPN, you'll work independently while being supported by an experienced clinical team. You'll build meaningful relationships with patients and families, make a direct impact on patient outcomes, and enjoy variety in your day-to-day work. Alternate Solutions Health Network collaborates with health systems in joint venture partnerships to create a post-acute care solution to deliver efficient centralized operations that is patient focused. Requirements
- Current Licensed Practical Nurse (LPN) license
- Minimum one year of acute care experience required
- Valid driver's license and active auto insurance
- Ability to travel throughout the assigned territory
Nice To Haves
- Home health experience preferred
- Strong assessment, communication, and critical thinking skills
Responsibilities
- Provide skilled nursing care in patients' homes under physician orders and RN direction
- Assess and monitor patient conditions
- Administer medications and treatments
- Educate patients and families on disease management and preventative care
- Collaborate with physicians, case managers, and interdisciplinary team members
- Complete timely clinical documentation in accordance with agency and Medicare guidelines
- Help reduce hospital readmissions through patient education and proactive care
