Licensed Practical Nurse (Nights)
About The Position
This role involves providing skilled nursing care to patients at the end of their life, conducting visits in facilities and/or home settings. The Licensed Practical Nurse will also provide on-call services after hours and complete skilled visits. This position assists Registered Nurses in carrying out the plan of care and performing specialized procedures as directed. The nurse will actively participate in interdisciplinary care planning, collaborate to ensure the patient plan of care is current, and address identified issues. Additionally, the role includes reinforcing patient and family education regarding end-of-life care and assuming responsibility for personal and vocational growth through participation in nursing organizations and in-service programs. The nurse will keep the RN Case Manager informed of changes noted during patient care.
Requirements
- Licensed Practical Nursing Degree
- A minimum of 1 year of skilled care experience
- Valid driver's license and Auto insurance Coverage
- Reliable transportation: Willing to travel

- Hospice or home health experience preferred
Responsibilities
- Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
- Provide on-call services after hours, and complete skilled visits.
- Assist Registered nurses in carrying out the plan of care.
- Assist RN in performing specialized procedures( as defined in the state nurse practice act) as directed by the RN under physician's orders.
- Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
- Assist in the reinforcement of patient and family education regarding end-of-life care.
- Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
- Keep the RNCM informed of changes noted during the process.
- Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research reading.
