About The Position

Works as part of a care team to improve the execution of quality requirements, increase throughput, improve efficiency, and enhance the overall patient experience. Provides care to designated patients and coordinates care across the continuum. Performs administrative functions in the practice.

Requirements

• Technical School Diploma from an LPN program.
• Ability to establish and maintain control of violent patients or dangerous situations.
• Basic computer skills, critical thinking, and ability to concentrate.
• Knowledge and competence reflecting current nursing practice.
• American Heart Association Basic Life Support certification for Pennsylvania, required upon hire.
• Licensed Practical Nurse (LPN) license for Pennsylvania, required upon hire.

Responsibilities

• Assist providers by collecting patient history and the reason for the visit.
• Complete observation, physical exams, telephone triage, and analysis of diagnostic data.
• Track and log ordered laboratory and diagnostic tests, and ensure providers receive all test results.
• Educate patients, ensure education materials are current and appropriate, and evaluate the effectiveness of patient education.
• Assist patients with prescription refill inquiries, appointment scheduling, and referral authorization.
• Assist providers during procedures and examinations and clean instruments and rooms.
• Coordinate internal and external appointments for new and established patients based on medical needs, preferences, and the practice schedule.
• Intervene to prevent, minimize, or correct actual or potential patient risks based on observation of physiological, behavioral, and environmental factors.
• Participate in telephone triage by obtaining clinically appropriate data and symptoms, and advise and instruct patients according to established protocols and/or provider orders.
