About The Position
We are seeking a dependable and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care in a Long-Term Care setting. The ideal candidate will be patient-focused, reliable, and committed to delivering exceptional resident care while working collaboratively with an interdisciplinary healthcare team.
Requirements
- Active Pennsylvania LPN License
- Current BLS Certification
- PA Department of Aging Fingerprint Clearance Letter
- Government-issued Photo ID
- Updated Immunization Records (per facility requirements)
- Strong communication and clinical assessment skills
- Ability to work independently and as part of a team
Nice To Haves
- Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience preferred
Responsibilities
- Deliver direct nursing care to long-term care residents
- Administer medications and treatments as prescribed
- Monitor resident conditions and report changes appropriately
- Complete accurate and timely documentation
- Assist with admissions, discharges, and resident assessments
- Ensure compliance with infection control and safety protocols
- Collaborate with CNAs, RNs, physicians, and rehabilitation staff
- Provide resident and family education regarding care plans and treatments
- Participate in care planning and interdisciplinary team meetings
- Maintain a safe, compassionate, and supportive care environment
