In a team relationship with the Registered Nurse, the Licensed Practical Nurse contributes to the nursing process through on-going systematic data collection, assisting with the health assessment, recording observations, goal identification and patient health education. The Licensed Practical Nurse will also collaborate with the healthcare team, ensure health and safety compliance, and perform clinical tasks. Performance Expectations include contributing to nursing assessment, care, and planning in conjunction with the RN supervisor. This role initiates the admission process, assists with identifying recurrent health problems, symptoms, and behavioral changes. The LPN collects pertinent data through observation, interview, and communication with the health team, recording all data in the patient's health record. They assist in forming a written plan, identifying short and long-term goals, and setting priorities for patient care. Additional duties include assisting the physician in minor medical/surgical procedures, transcribing and posting therapeutic orders, administering medications, practicing universal precautions, and maintaining a safe environment. Confidentiality and patient privacy are paramount. The LPN must maintain active state nursing licensure, adhere to the nursing code policy, and present a neat appearance, including wearing a white lab coat. Punctuality for shifts and verbal reports is expected. The role requires adherence to policies regarding absences, illness notification, vacation requests, and workday exchanges. The LPN must also maintain security clearance and conform to the dress code.
