About The Position

This is a Temporary Part Time position for a Licensed Practical Nurse (LPN) at Carewest's Garrison Green location, specifically within the EQuaL program. The LPN reports to the Director of Care and is a key member of the interprofessional health care team. Responsibilities include conducting client assessments, developing and implementing care plans that address physical and psychosocial needs, monitoring client conditions, administering medications, supervising unregulated staff in medication delivery, and collaborating with the interdisciplinary team. The role requires a strong foundation in trauma-informed care and experience with individuals who have mental health diagnoses, responsive behaviors, and neurological conditions such as brain injury, spinal cord injury, and multiple sclerosis (MS). The LPN will also support younger adults with chronic diseases and disabilities, who may have complex and evolving care requirements. This position provides guidance and leadership to Health Care Aides (HCAs) and must maintain professional and ethical standards in accordance with CLHA standards, Carewest policies, and other relevant legislation. The EQuaL program serves adults aged 18 to 64 with disabilities related to diseases or conditions like MS or brain injury, requiring continuing care support.

Requirements

  • Graduation from a recognized LPN program.
  • Current and active registration with CLHA required.
  • Current and active Basic Life Support Provider CPR is required.
  • Demonstrated leadership skills required.
  • Adherence to relevant privacy legislation and best practice including HIA, ATIA, POPA, and InfoCare Behaviours, is required.
  • Successful applicants not currently working for Carewest will need to provide a suitable police records check and vulnerable sector screen.
  • Successful applicants may need to provide immunization records for positions in certain environments.

Responsibilities

  • Completes comprehensive client assessments.
  • Develops and implements client plans/care that addresses both physical and psycho-social care needs.
  • Monitors on-going client care and condition to ensure optimum functioning.
  • Ensures changes in client status are reported to the Staff Nurse.
  • Administers prescribed medications.
  • Supervises the delivery of medications by unregulated staff.
  • Collaborates with the interdisciplinary team.
  • Provides guidance and leadership to HCAs in the delivery of care.
  • Maintains professional and ethical standards at all times.
  • Responsible for other related duties as required.
