About The Position

Under the direction of a Licensed Medical Professional, the Nurse is responsible for working with the Psychiatric Providers and the Clinical Staff to assist in the intake and maintenance of Psychiatric Clinic clients. The Nurse will also provide support functions to the providers.

Requirements

  • Licensed as an LPN or RN in the state of Oregon.
  • At least 2 years’ experience in professional nursing field work is required.
  • Valid Oregon driver’s license, clean driving record.
  • Personal automobile insurance is required if clients are transported or any agency business is conducted by the employee using his/her personal vehicle.
  • Must possess an active RN or LPN Nursing license in the State of Oregon.
  • Understanding of basic principles of mental health and mental illness.
  • Knowledge of common mental health disorders and their symptoms.
  • Familiarity with developmental stages and their impact on mental health.
  • Skills in establishing rapport and building trusting relationships with clients.
  • Ability to provide emotional support and encouragement.
  • Competence in engaging clients in their treatment plans and goals.
  • Ability to observe and document client behaviors, symptoms, and progress.
  • Skills in conducting basic assessments under supervision.
  • Understanding of when to seek supervision or refer to higher-level professionals.
  • Competence in coordinating care and services for clients.
  • Knowledge of community resources and how to access them.
  • Ability to work as part of an interdisciplinary team.
  • Skills in implementing basic therapeutic interventions and support strategies.
  • Ability to respond appropriately to crises and emergencies.

Responsibilities

  • Provide telephone and in person triage independently within scope of practice and make appropriate clinical decisions.
  • Implement individualized care planning for those with limited resources and/or limited learning abilities.
  • Provide basic health screenings, which may include wound care and other medical assessments.
  • Understand and follow up on abnormal results of labs upon consultation with Licensed Medical Professional.
  • Coordinate with community providers to obtain needed medical supports such as care-givers, durable equipment purchases and maintenance.
  • Coordinate and consult with PCP, medical specialists, hospitals, rehabilitation programs and other services on behalf of mutual patients.
  • Provide in-home skills training to patients to monitor and effectively manage medical conditions such as diabetes, COPD and Hepatitis.
  • Administers medications and injections as prescribed by the LMP.
  • Screens participants for medication side effects and medical conditions.
  • Provide individual and group skills training regarding nutrition, medications, and physical wellness as components of mental health recovery.
  • Participate in trainings and meetings.
  • Provide education and training to team members regarding medications and potential side effects.
  • Maintain the medication room to proper OAR standards and administration best practices.
  • Complete required documentation within the specified timelines stated below under “Documentation Requirements.”
