About The Position

This is a casual Licensed Practical Nurse position reporting to the Manager, Recreation and Therapeutic Programs. The Licensed Practical Nurse works in conjunction with the Manager in the day-to-day operations of the Adult Day Program. The role involves supporting the NVS Model of Service, Mission, Vision, and Values, including Person-Centered Care. Key responsibilities include managing client needs, administering medications and treatments, monitoring client records, participating in health and wellness strategies, supervising staff, coordinating the interdisciplinary team, assisting with staff selection and orientation, collecting and reporting utilization indicators, managing supplies, participating in quality improvement initiatives, and monitoring safety.

Requirements

  • Graduate of a recognized Practical Nursing Program
  • Practising member of the College of Licensed Practical Nurses of BC, holding a valid BC Practical Nurse license in good standing
  • One year recent related experience or an equivalent combination of education, training and experience.
  • One year experience specific to long term care practice.
  • Ability to communicate effectively both verbally and in writing
  • Ability to deal with others effectively
  • Ability to organize work
  • Ability to operate related equipment
  • Physical ability to carry out the duties of the position

Responsibilities

  • Supports the NVS Model of Service, Mission, Vision, Values including Person Centered Care interpreting to assigned staff.
  • Manages vacancies and wait lists and liaises with Home Health to arrange for new clients to come into the Adult Day Program. Manages the information flow regarding deaths and discharges.
  • Ensures client’s needs are identified and that appropriate goals are established and achieved through assessment/ care planning / appropriate interventions and continuous evaluation. Consults with Clinical Lead for interventions outside scope of Practice.
  • Performs direct care such as administration of medications, treatments and personal care.
  • Monitors Client Records to ensure they are complete and accurate and in compliance with professional standards.
  • Participates with campus outreach team to identify and deliver outreach health and wellness strategies to clients, to support identified needs and priorities.
  • Provides supervision to the rehabilitation assistant and provides guidance to the activity aide, under the direction of the Director of Care.
  • Assists the Manager with the coordination of the interdisciplinary team who provide care and services to the Day Program Clients. The team includes staff, physicians, visiting professionals, contracted therapists, student placements and volunteers.
  • Participates in the selection process for new staff as requested by the Manager.
  • Assists the Manager by orientating new staff and providing feedback to the Manager for the purpose of performance evaluations. Liaises with Staffing Clerk to ensure appropriate staffing, and coordinates work assignments, within the parameters of the Collective Agreement.
  • Collects, analyzes and reports utilization indicators within the organization and to external funding bodies. Continually evaluates, utilizes and shares Best Practice information / research to improve client outcomes.
  • Monitors product usage and use of supplies by reporting purchases for the Program to the Manager and ordering supplies when required.
  • Participates as a member of the Continuous Quality Improvement committee and supports quality improvement activities in the organization.
  • Monitors safety in the working and living environment. Responds to and reports all hazards and incidents; investigates and makes recommendations as indicated.
  • Participates in ensuring the safety of our residents by reporting observations related to unsafe circumstances for a resident to the appropriate care staff.
  • Provides input to and supports the Strategic Plan of the organization by attending Strategic Planning Sessions when requested.
  • Other related duties as assigned.
