About The Position

The Retreat Licensed Practical Nurse is responsible for age-appropriate assessment, planning, implementation, and evaluation of residents. The role is accountable for ensuring that planned care is delivered in a safe, economical, and efficient manner, adhering to facility policies and procedures. This position functions as an important member of the nursing team, receiving assignments from and reporting to supervisor(s).

  • Ability to complete incident reports, event fall forms, and observation forms.
  • Ability to administer CPR.
  • Ability to complete Report Sheet and Charge Nurse Checklist.
  • Ability to update the schedule and daily assignment.
  • Ability to assist physicians.
  • Ability to ensure 2-step PPD is done on admission and results documented.
  • Ability to order OTC medications and keep med room organized.
  • Ability to assist DON, ADON, and Administrator with duties and projects.
  • Ability to maintain confidentiality of records and issues.
  • Ability to receive visitors, answer telephone, and answer call lights courteously.
  • Ability to fax pharmacy reorder sheets.
  • Ability to assure residents receive meals and are assisted with meals.
  • Ability to receive, verify, and put away medications.
  • Ability to communicate with doctors and family members regarding changes in residents.
  • Ability to record I & O’s in the EHR.
  • Ability to carry out physician orders and notify physician if an order cannot be carried out.
  • Ability to document glucose levels in the EHR and accu check book and notify physicians.
  • Ability to stock medication carts.
  • Ability to arrive for shift no earlier than 15 minutes before and clock out no later than 15 minutes after scheduled shift.
  • Willingness to work weekends on a rotating basis.
  • Willingness to work one overtime shift a month if needed.
  • Demonstrates loyalty, honesty, and integrity.
  • Demonstrates positive support of programs/projects.
  • Maintains confidentiality of patient/resident information.

Responsibilities

  • Receive report and count with off-going nurse; be prepared to give report to the on-coming nurse unless in an emergency.
  • Comply with State and Federal Rules and Regulations for long term care facilities, and follow facility policies and procedures.
  • Supervise, teach, and assist C.N.A.s as necessary, ensuring residents are turned and positioned, cleaned, and dried.
  • Acquire general knowledge of pertinent facility and nursing policies and other department's functions and interactions with nursing service.
  • Function as an important member of the nursing team, receiving assignment from and reporting to supervisor(s) as indicated.
  • Perform work accurately, neatly, and completely.
  • Maintain organization at the nurse’s station.
  • Communicate resident concerns, problems, and/or changes in condition to the physician.
  • Provide privacy for all residents.
  • Complete admissions, discharges, and transfers of resident forms as needed.
  • Answer resident’s call light and seek assistance as necessary.

  • Administer medications as ordered with each medication pass and document in the EHR.
  • Complete treatments as ordered and document in the EHR, if indicated.
  • Check to assure the C.N.A.s have completed all duties and POC by the end of the shift.
  • Document all pertinent information in the EHR.
  • Complete incident reports, event fall forms, and observation forms in the EHR each shift and report to DON/ADON as necessary.
  • Administer CPR when indicated and know how to do so.
  • Complete Report Sheet and Charge Nurse Checklist each shift.
