Requirements

  • Current and valid New Jersey Practical Nurse license required
  • An active BLS/CPR certification from the American Heart Association is required
  • LPN experience required

Nice To Haves

  • Wound care and hyperbaric experience preferred
  • HBO experience preferred
  • Outpatient experience preferred

Responsibilities

  • Reports timely and accurate changes in the patient's condition. Change in vital signs and blood sugars. Turns in assignment sheet for evaluation. Change in pain status, etc.
  • Demonstrates knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Follows plan of care as directed by the Registered Nurse. Examples may include but are not limited to: properly complies with appropriate physician orders. Observes, reports and assists in meeting patient needs as per plan of care. Observes and reports the patient's physical and mental status. Provides medical treatments as per physician order. Administers prescribed medication. Records vital signs, intake and output other related responsibilities. Supports/reinforces patient teaching and discharge planning. Assists patients with personal hygiene, nutrition and daily activities. Appropriately utilizes and cares for technical equipment and supplies. Assists physicians with medical treatments, exams and surgical procedures appropriate to the unit. Maintains safe and therapeutic care environment.
  • Appropriately participates during crisis situations. Isolates and removes other patients and family members. Assists with and reassures patients and families. Stays with a patient in need. Assists with and obtains medical equipment and supplies as needed.
  • Special care is taken to ensure all belongings are properly gathered, labeled with patient identification and are sent with the patients who are discharged, transferred or expire. Valuables are placed in safe as per policy. Assignments are accurately completed with appropriate supervisory counseling. Completes patient related activities within scheduled time periods.
  • Clinical practice accurately reflects Code of Ethics, Board of Nursing and the New Jersey Department of Health regulations, JCAHO and institutional policies and procedures and promotes professional ethics/standards.
  • Data collection, plan of care and intervention of patient care are reflective of Policy and Procedure and Standard of Care, measured by complete and accurate documentation. Utilizes data information systems appropriately.
