Requirements
- Current Licensed Practical Nurse (LPN) license
- Minimum of one year of acute care LPN experience
- Valid driver's license and active auto insurance
- Strong assessment, communication, and documentation skills
- Compassionate, dependable, and patient-focused
Nice To Haves
- two years preferred
- Home health experience preferred
Responsibilities
- Providing skilled nursing visits in patients' homes
- Monitoring patient conditions and communicating changes to the care team
- Administering medications and treatments
- Educating patients and families on disease management and preventative care
- Helping reduce hospital readmissions through quality patient education
- Collaborating with physicians, case managers, and interdisciplinary team members
- Completing timely documentation in accordance with Medicare and agency guidelines
