Requirements

  • Current LPN (Licensed Practical Nurse) license in New York State
  • Experience in long-term care, skilled nursing, or geriatric settings preferred
  • Strong ability to lead and support CNAs and care teams
  • Clear communication skills — both verbal and written
  • A compassionate, respectful approach to resident care
  • Ability to work in a fast-paced, hands-on environment
  • Commitment to delivering care aligned with high clinical and regulatory standards
  • This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.

Responsibilities

  • Administer medications and treatments safely and accurately (following the “5 rights”)
  • Lead and support CNAs in delivering high-quality resident care
  • Monitor resident conditions and report changes to nursing leadership
  • Follow and implement care plans to ensure consistent, individualized care
  • Maintain accurate clinical documentation (MARs, treatment records, nursing notes)
  • Ensure safe handling of medications, including controlled substances
