Requirements

• Graduate from an accredited school of nursing.
• Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment.
• Must be able to obtain and maintain CPR certification.

Responsibilities

• Provide comprehensive, intensive clinical services to members enrolled in the MAT program, both on-site and off-site.
• Deliver medical and support services such as wellness education, administering injections, coordinating labs and pharmacy services, monitoring vitals, and facilitating communication with psychiatric and medical providers.
• Ensure that members receive the appropriate medical care and support throughout their treatment journey.
• Assist in assessing physical, psychological, and social aspects of patients and plan individualized treatment programs in consultation with RNs and other staff.
• Carry out individualized treatment programs, including administering medications, obtaining diagnostic tests, and assisting in medical or minor surgical procedures as directed.
• Adhere to established policies, procedures, and protocols, including counting controlled substances, using proper safety techniques, and responding to health emergencies.
• Document nursing encounters using SOAP charting and communicate essential information to relevant healthcare staff.
• Attend mandatory staff meetings and training sessions to stay updated on healthcare practices and protocols.
