About The Position

The Licensed Practical Nurse (LPN) provides direct nursing care to participants in CCH's Recuperative Care programs within the Colorado LPN scope of practice. The LPN performs both independent and delegated duties under the supervision and delegation of CCH Registered Nurses (RNs), contributing to patient assessment, medication administration, wound care, treatments, activities of daily living, and health education. The LPN works as part of an integrated, multi-disciplinary care team to deliver trauma-informed, harm-reduction care in a 75-bed, 24/7 recuperative care setting, and supports safe, timely documentation and coordination of care across CCH programs.

Requirements

  • Current Colorado LPN license in good standing required.
  • Basic Life Support Certification required within 30 days of employment.
  • Reliable transportation is required.
  • A passion for working with vulnerable populations with a focus on trauma-informed care.
  • A passion for working in a harm-reduction environment with a focus on collaborative goal planning with patients.
  • Sound clinical judgment within the LPN scope of practice and the ability to recognize and escalate changes in patient condition.
  • Strong interpersonal and communication skills with high emotional intelligence.
  • Adaptability, resilience, and a proactive approach to clinical work in a 24/7 facility.
  • Knowledge of, and sensitivity to, the needs of people experiencing homelessness and other vulnerable populations.
  • Proficiency in EHR use and documentation compliance.
  • Commitment to universal precautions and regulatory compliance (OSHA, HIPAA).

Nice To Haves

  • Bi-lingual English-Spanish and/or ASL preferred
  • Minimum one (1) year of nursing experience in a clinical, community health, or acute care setting preferred.
  • Experience working with people experiencing homelessness or other diverse and marginalized populations is highly desirable.
  • Experience in harm-reduction environments is preferred.

Responsibilities

  • Deliver safe, effective nursing care in accordance with the Colorado LPN scope of practice, CCH protocols, and the direction of the RN or provider, within an integrated, trauma-informed, harm-reduction model of care.
  • Perform plan of care interventions, including specimen collection, wound care, and other clinical needs, following written protocols or direct provider order.
  • Administer medications following written protocols or direct provider order, and understand the limitations of medication administration within LPN scope.
  • Assist patients with performing activities of daily living, including personal hygiene, elimination, nutrition, mobility, and ambulation, when needed.
  • Empower patients by providing appropriate ongoing and reinforced health education and information after the initial RN education plan is created and provided, including but not limited to diabetes management, hypertension management, nutrition, wound care, and disease prevention for the common chronic and acute conditions seen in this population.
  • Collaborate with members of the multi-disciplinary health care team, including registered nurses, medical assistants, primary care medical providers, behavioral health providers, psychiatric providers, peer specialists, dental assistants and hygienists, pharmacists, patient navigators, case managers, client access representatives, inventory and lab coordinators, and nurse educators, to ensure holistic and integrated support.
  • Provide nursing services as needed for other CCH programs.
  • Serve as a preceptor to new staff members and nursing students as assigned.
  • Perform other duties as assigned.
