About The Position

Established in 1970, Boley Centers, Inc. is a private, non-profit organization serving individuals with mental disabilities, individuals and families who are homeless, Veterans and youth in Pinellas County. Boley Centers, Inc., a nationally recognized and accredited behavioral health care agency, has an opening for a full-time Licensed Practical Nurse (LPN) to work with our Florida Assertive Community Treatment (FACT) team. The FACT LPN will be responsible for conducting physical health assessments and assessing physical health needs, making appropriate referrals to community physicians and providing administration and management of medication in conjunction with psychiatrists. The FACT LPN also provides a range of case management, education, treatment, rehabilitation and support services to assigned consumers. Due to the nature of the FACT program being 24/7, the FACT LPN may be required to work outside business hours, weekends, holidays and on-call.

Requirements

  • High school diploma/GED required.
  • Current Licensed Practical Nurse (LPN) license required.
  • At least one year of psychiatric nursing experience in an in-patient or out-patient mental health setting, working with SPMI clients or substance abuse treatment.
  • Knowledge of mental health counseling, coordinating home health care, severe and chronic mental disorders.
  • Must be able to pass Level 2 background check and drug screen.
  • Valid Florida driver's license, own transportation vehicle and vehicle insurance required.
  • Current LPN licensure in Florida required.

Responsibilities

  • Conducts initial physical health assessments.
  • Initiate, update and revise treatment goals and plans utilizing team input and provides case management services when required.
  • Administer and monitor medication regimen and manage pharmaceuticals and medical supplies.
  • Liaise with community agencies and families to coordinate treatment and services.
  • Perform on-call emergency crisis intervention.
  • Act as shift manager in coordination with other FACT Team members.
  • Educate consumers regarding medication compliance and potential side effects and evaluate and follow up on any medical concerns.
  • Operate own, or agency vehicle, to transport clients and/or travel between locations.
  • Attend and participate in daily staffing and treatment planning meetings.
