About The Position

This position will travel between the Kokomo/Peru/Lafayette areas. We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in New Castle, IN. This is a case management role with semi-annual physical assessments, typically 8am-5pm, with a focus on low stress, relaxed, high morale, family-like, team-oriented environment. The role involves ensuring each individual receives quality, individualized health care management, conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress. The LPN will also act as a health/medical resource for staff, training staff in the medical arena, conducting audits and reviews of medications, and ensuring company compliance with state requirements and organizational policies regarding medical care and records.

Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Indiana required
• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
• Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
• Strong verbal/written communication skills
• Ability to work independently and efficiently
• Organizational and computer skills
• Acceptable background check & driving record check

Responsibilities

• Ensure that each individual receives quality, individualized health care management, resulting in their optimal health
• Conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress
• Act as a health/medical resource for staff, training staff in the medical arena
• Conduct audits and reviews of medications
• Ensure that the company is in compliance with state requirements and organizational policies regarding medical care and records
• Travel between the Kokomo/Peru/Lafayette areas
• On call for triage of emergencies one week per month
