About The Position

Alvita Care is seeking experienced, CT-licensed LPNs whose first language is English and who also speak an additional language (Bilingual) for private duty nursing services. This is a per diem role for Western Connecticut (Fairfield, New Haven, Litchfield County). Ideal candidates are reliable, compassionate, and willing to deliver consistent, one-on-one care. We offer flexible hours with shifts ranging from 6, 8, to 12 hours, available for both day and night shifts. You can work with us part-time, per diem, or around your current work schedule. Shifts are assigned to provide 1:1 care in the home environment. Alvita Care is a mission-driven Home Healthcare & Personal Care Agency with four branches and over 500 caregivers, providing relief and assurance to family members in areas such as In-Home Personal Care, Alzheimer's & Dementia Care, Palliative & Hospice Care, and Geriatric Care Management. We have earned The Joint Commission Gold Seal of Approval. Our clients are all private-pay, and we do not accept Medicare or Medicaid. We are expanding our services in Connecticut and seeking top-notch caregivers.

Requirements

  • Experienced, CT-licensed LPN, Licensed Practical Nurse
  • First language is English
  • Speak an additional language (Bilingual)
  • Minimum of 6 months to (1) year demonstrated experience as an LPN
  • Ability to put patients at ease and explain physician's instructions.
  • Ability to multi-task with excellent organization skills.
  • Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
  • Knowledge of medical terminology.
  • Team player and ability to work independently
  • Valid driver's license or Non-Net Driver photo ID card

Nice To Haves

  • Drivers preferred!
  • BLS, ACLS, or CPR certification.

Responsibilities

  • Assess needs of the clients to develop a plan of care
  • Consult with Management on complex/unique cases
  • Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients/patients/
  • Monitor/Assess patients/clients' condition, effects of the care plan,
  • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
  • Manages, plans, coordinates and executes the patient's safe discharge from service.
  • Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.
  • Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
  • Assists patients with activities of daily living
  • Documents content and process of all services provided in internal system properly.
  • Transport/utilize medical equipment to and from clients/patients' home/facilities.
  • Travel to patient/clients' homes and/or other facilities.
  • Perform other related duties and participates in special project, as assigned.
