About The Position

Cadia Healthcare is seeking a Licensed Practical Nurse (LPN) to join their team. LPNs play an essential role in the facilities, and their skills are vital to the care provided, making a difference in patients’ lives every day. By directing the duties of nursing assistants and performing day-to-day nursing duties, LPNs ensure residents are fully cared for. Requirements

  • Must possess a current, unencumbered, active license to practice as an LPN in this state or compact state.
  • Must possess a current CPR license.

Nice To Haves

  • Previous long-term care experience is desirable.

Responsibilities

  • Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations & guidelines that govern the long-term care facility.
  • Meet with assigned nursing assistants to give a Nursing Report on assigned residents and review 24-hour summary reports in the electronic medical record on assigned residents.
  • Perform administrative duties, such as completing medical forms, evaluations, observations, events & appropriate documentation for all assigned residents.
  • Ensure all medications & treatments have been signed off prior to the end of the assigned shift and update the Nursing Supervisor on resident changes in condition.
  • Receive telephone orders from physicians, transcribe orders & record in the electronic health record, and prepare and administer medications and treatments as ordered by the provider.
  • Inform nursing staff of new admissions, their expected time of arrival, room assignment, etc.
  • Ensure that rooms are ready for new admissions, greet residents upon admission, and escort them to their rooms as necessary.
  • Make rounds with providers as necessary and make periodic checks to ensure resident care is being completed by CNA/GNA & evaluate resident status.
  • Ensure that nurse’s notes reflect that the care plan is being followed when administering nursing care or treatment and review care plans for appropriate resident goals, problems, approaches, & revisions based on nursing needs.
  • Update care plans as appropriate.
