About The Position

This position will travel between the Kokomo/Peru/Lafayette areas. We are seeking a Licensed Practical Nurse (LPN) to help support the continuous enrichment and development of individuals with developmental disabilities in our adult residential facilities in New Castle, IN. This is a case management role with semi-annual physical assessments, typically 8am-5pm, Monday thru Friday, with flexible hours. It is a low stress, relaxed, high morale, family like, team oriented environment. The role involves ensuring that each individual receives quality, individualized health care management, resulting in their optimal health. This includes conducting routine site visits, checking vitals, doing assessments, counseling of medication administration, and documenting progress. The LPN will act as a health/medical resource for staff, training staff in the medical arena, and conducting audits and reviews of medications. The position also ensures that the company is in compliance with state requirements and organizational policies regarding medical care and records. The LPN will be on call for triage of emergencies for 1 week per month.

Requirements

• Current Licensed Practical Nurse (LPN) license in the State of Indiana required
• Must be skilled, flexible, self-motivated, dependable, and able to work cooperatively with staff, interdisciplinary team members, and outside agencies
• Strong verbal/written communication skills
• Ability to work independently and efficiently
• Organizational and computer skills
• Acceptable background check & driving record check

Nice To Haves

• 1-2 years experience working with individuals with disabilities or other long-term care is preferred
