Requirements
- Current Massachusetts LPN license
- Graduate of an accredited nursing program
- At least one year of home care experience
- Valid driver’s license, reliable transportation, and current auto insurance
- Ability to travel throughout Malden, Lowell, and surrounding communities
- Ability to sit, stand, bend, lift and move intermittently and lift 50–100 lbs.
Nice To Haves
- Home health or hospice experience preferred
Responsibilities
- Provide skilled nursing care under the direction of an RN and physician
- Follow individualized plans of care and physician orders
- Monitor patients and report changes to the RN Case Manager
- Educate patients and caregivers on medications, treatments, and self-care
- Complete accurate and timely clinical documentation
- Collaborate with the care team to improve outcomes and help prevent avoidable hospital visits
