About The Position

Symbii Home Health and Hospice is seeking a Licensed Practical Nurse (LPN) to join their team. This role offers the opportunity to provide life-changing service to patients in southeastern Idaho. Requirements

  • Must possess a valid LPN license in the State of Idaho
  • Must hold a current CPR certification
  • Must hold a valid driver’s license and current auto insurance

Nice To Haves

  • Desire to provide LIFE CHANGING SERVICE to your patients
  • Experience in wound care, IV infusions, injection and medication administration, and patient/caregiver education

Responsibilities

  • Provide direct patient care as defined in State’s Nurse Practice Act.
  • Implement plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s).
  • Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
  • Participate in personal and professional growth and development.
  • Perform other duties as assigned by the registered nurse.
