LICENSED PRACTICAL NURSE (LPN)

Requirements
- Graduation from an accredited nursing program
- Current LPN license in the State of Wisconsin (must be maintained)
- Ability to supervise Nursing Assistants
- Strong clinical skills, compassion, and commitment to resident-centered care
- Must pass a Wisconsin Caregiver Background Check
- Valid Wisconsin driver’s license (if driving is required)

Responsibilities
- Provide high-quality nursing care under the supervision of a Registered Nurse
- Administer medications and treatments in accordance with the Nurse Practice Act
- Observe residents, document care, and report changes in condition
- Assist in developing and implementing individualized care plans
- Support training and orientation for new staff
- Ensure a safe environment for residents and staff
- Uphold residents’ rights, privacy, and person-centered care principles
- Create a positive, respectful, and compassionate experience for residents, families, and colleagues
- Act with integrity, honesty, and respect
- Set goals, drive results, and hold themselves accountable
- Foster teamwork and help others grow
- Communicate effectively and build meaningful relationships
