Interim HealthCare is currently seeking compassionate and skilled Licensed Practical Nurses (LPNs) for a skilled home health position based out of our Manchester, NH office and serving the surrounding communities. In this role, you will provide individualized nursing care to patients in their homes while working independently with the support of an experienced and collaborative healthcare team. Requirements

  • Current Licensed Practical Nurse (LPN) license in the State of New Hampshire
  • Current CPR Certification required (Monthly renewal classes available and paid for by Interim HealthCare)
  • Reliable transportation
  • Strong clinical assessment and communication skills

Nice To Haves

  • Minimum of two (2) years of skilled nursing or acute care experience preferred

Responsibilities

  • Provide skilled nursing care and treatments to patients in their homes
  • Monitor patient conditions and report changes to supervising clinicians and physicians
  • Administer medications and treatments as prescribed
  • Assist with patient assessments and individualized care planning
  • Maintain accurate and timely clinical documentation
  • Educate patients and family members on disease management, medications, treatments, and safety practices
  • Collaborate with interdisciplinary healthcare professionals to support patient goals and quality outcomes
  • Ensure compliance with all company policies and state and federal healthcare regulations
