Make a Difference Every Day We're seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN) to join our team and help patients navigate their healthcare journey with confidence. In this role, you'll work closely with providers, patients, pharmacies, and care teams to coordinate medication management, provide clinical support, and ensure patients receive the timely, high-quality care they deserve.

Requirements

  • Current Licensed Practical Nurse (LPN) license in good standing
  • Strong communication and patient service skills
  • Proficiency with EMR systems and Microsoft Office applications
  • Ability to multitask and manage competing priorities in a fast-paced healthcare environment
  • Strong attention to detail and commitment to accurate documentation
  • Excellent telephone triage and critical-thinking skills
  • Ability to build rapport with patients, providers, pharmacies, and community partners
  • Team player who thrives in a collaborative Patient-Centered Medical Home (PCMH) environment
  • Current Colorado LPN license
  • 6 months or greater working as an LPN in a clinical setting

Responsibilities

  • Manage prescription refill requests and medication-related patient communications
  • Maintain accurate medication records, including prescriptions, over-the-counter medications, and supplements
  • Process prescription prior authorizations and assist with PDMP reporting
  • Coordinate medication refill protocols and route requests to providers for review and approval
  • Assist patients with scheduling appointments when medication evaluations or follow-up care are needed
  • Maintain current controlled substance agreements and related documentation
  • Perform telephone triage to assess patient concerns and determine appropriate scheduling or follow-up
  • Support patient communication through phone calls, patient portal messages, and outreach efforts
  • Verify and update patient demographic, insurance, pharmacy, and emergency contact information
  • Schedule appointments and connect patients with appropriate services and resources
  • Document all patient interactions and clinical activities within the electronic medical record (EMR)
  • Participate in quality improvement initiatives and team-based care activities
  • Assist with clinic operations and support other departments as needed
