About The Position

Center for Human Development (CHD) is seeking Per Diem Behavioral Health Licensed Practical Nurses (LPN) to join the team at Heron House, an Enhanced Medical Group Living Environment (EMGLE) program. This 18-bed facility in East Longmeadow, MA, offers a low LPN to client ratio, allowing for quality care and support. Under the supervision of a designated charge nurse or RN, you will collaborate with a support team of LPNs, CNAs, and Direct Care staff, providing compassionate care and gaining valuable experience in CHD's modern EMGLE facility. The EMGLE program provides skilled healthcare and supportive services, including around-the-clock nursing and personal care assistance, to individuals with mental illness and compounding medical needs. These services aim to meet daily needs, support individuals with chronic medical conditions, terminal illnesses, and/or disabilities impacted by significant mental illness, and prevent hospitalization or nursing facility admissions. The role offers a competitive hourly wage of $38.50/hr, with a $2.00/hr differential for overnight shifts. The schedule is Per Diem, with all shifts open.

Requirements

  • Have a license in practical nursing by The Board of Registration in Massachusetts
  • Valid driver’s license
  • Meeting physical demands of the role. IE: Ability to lift 45 lbs. or more, stocking supplies, extended periods of walking and/or standing
  • Participate and successfully complete assigned training requirements in compliance with CHD and the Massachusetts Department of Mental Health

Nice To Haves

  • One-year experience as a LPN, preferred
  • One-year experience in a Behavioral Health setting, preferred
  • One-year experience working with the elderly, people with disabilities, behavioral challenges and cognitive disabilities, preferred
  • Strong written skills and experience with Electronic Health Record Management is a plus

Responsibilities

  • Medication administration and management. Assist in medication audit processes
  • Attend participants care plan meetings/resource meetings
  • Provide direct care as needed. Providing personal hygiene care by assisting with travel to the bathroom; helping with showers and baths
  • Effectively understand details about a patient’s health and provide charge nurse or RN with clear, precise information and observations
  • Provides activities of daily living by assisting with serving meals, feeding Persons as necessary, ambulating
  • Provides adjunct care by administering nonsterile dressings, surgical preps, ice packs, and heat treatments
  • Maintains the stability of Persons served by checking vital signs and weight, recording intake and output information
  • On call monthly rotation (stipend provided, plus hours wage if called in to pass meds)
