About The Position

Symbii Home Health and Hospice is seeking a Licensed Practical Nurse to join our team! This role offers the opportunity to provide LIFE CHANGING SERVICE to patients in their homes. Symbii Home Health and Hospice is described as the best Home Health and Hospice Agency in Southeastern Idaho, emphasizing flexible hours, minimal weekends, consistent hours, and greater life balance compared to hospital or facility settings. The position allows for direct, one-on-one patient care, fostering trusting relationships and enabling patients to improve health and independence. Nurses will learn and enhance skills in areas such as wound care, IV infusions, medication administration, and patient/caregiver education. While working independently, team members are always available for support. The LPN will be responsible for patient care decisions, collaborating with case managers, physicians, families, and other care team members. The role also involves developing relationships with other caregivers in facilities and physician's offices to coordinate care.

Requirements

  • Must possess a valid LPN license in the State of Idaho
  • Must hold a current CPR certification
  • Must hold a valid driver’s license and current auto insurance

Responsibilities

  • Provide direct patient care as defined in State’s Nurse Practice Act.
  • Implement plan of care initiated by the registered nurse.
  • Provide accurate and timely documentation consistent with the plan of care.
  • Collect assessment data and perform observation to report to and collaborate with RN(s) and physician(s).
  • Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing.
  • Participate in personal and professional growth and development.
  • Perform other duties as assigned by the registered nurse.
